An interesting opportunity has arisen for a Social Worker to join our Community Hospital (CH) and Community Support Beds (CSB) Discharge Team, supporting patient flow from community hospitals and CSB units across Derbyshire.

🏥 About the Role:

You’ll play a vital role in facilitating timely and person-centred discharges, initially focusing on Whitworth Hospital in Darley Dale, with flexibility to cover other units in Staveley, Buxton, and Ilkeston during planned or unplanned absences.

The services are going through an exciting period of change over 2025-2026 to improve services for people in Derbyshire. As part of improving collaboration between health and social care, the Community Hospital and CSB Discharge Team will be delivering their support within our new integrated partnership between Derbyshire County Council and Derbyshire Community Health Services NHS.

The post holder will be a key player in supporting health and care systems during a period of increasing pressure on our pathway 2 provision. The postholder will be required to form close relationships with hospital staff as key members of the Multi-Disciplinary Teams, working regularly within the hospital in line with plans for integration with Derbyshire Community Health Services. The office base for the post will be negotiable and mileage and travel time will be paid in accordance with the current travel policy.

  • Experience in assessment and reviewing is desirable.

  • Strong organisational and time management skills.
  • Knowledge of local resources to support sustainable discharges.
  • Excellent communication and interpersonal skills skills to both support the people and their families, whilst enabling you to liaise and maintain positive relationships with hospital staff, CSB Unit staff and workers from other disciplines.
