Are you passionate about making a difference in people’s lives? We’re looking for an enthusiastic and motivated Social Care Practitioner to join our friendly and supportive Adult Social Work Team in Derbyshire Dales South.

💼 About the Role:

As a Social Care Practitioner, you’ll carry out Care Act 2014 assessments and reviews for adults receiving care in the community. Using a strengths-based approach, you’ll help individuals maximise their independence, choice, and control by building on their personal strengths and community networks.

  • Strong communication skills to work collaboratively with individuals, families, professionals, and agencies.
  • Ability to travel for home visits.
  • Excellent IT skills and adaptability in a fast-paced environment.
  • Strong organisational skills and the ability to work independently.
