Due to retirement of the existing post holder, an exciting opportunity has arisen for a qualified Social Worker to join the Bolsover South Prevention and Personalisation (P&P) Team.
We work with vulnerable adults, including individuals transitioning into adulthood, who have long-term care and support needs. Our focus is on delivering person-centred, strengths-based practice. This includes completing Care Act assessments, mental capacity assessments, and best interest decisions where required, alongside developing support plans that promote independence and achieve individual outcomes.
The role involves visiting people in their own homes or care settings, working closely with individuals and their carers to gather information, make informed decisions, arrange appropriate support (including equipment, tech and care), and maintain accurate records. You will also undertake safeguarding enquiries in line with Making Safeguarding Personal principles.
Our team consists of Social Workers, Community Care Workers, a Senior Practitioner, and a Service Manager. We work collaboratively with partner organisations and professionals, and team members are linked with GP surgeries to support effective multi-disciplinary working.
We offer flexible working arrangements, including a combination of home and office-based working. You will receive regular support through:
- Daily team catch-ups via Microsoft Teams
- Monthly one-to-one supervision
- Monthly team meetings
The team also operates a ‘champion’ model, providing opportunities to develop specialist interests, enhance practice knowledge, and share expertise with colleagues.
