If so, Derbyshire County Council Adult Social Care and Health wants to hear from you.  We are seeking to recruit a Social Worker permanent full-time based at The Quad in Chesterfield.  

As part of our Short Term Assessment and Reablement Team (START), you will be required to take a strength-based focused approach to deliver a proportionate, personalised, safe and sustainable response for local adults, their carers, and families. You must be able to work autonomously in a multi-disciplinary team. You will work alongside the Enablement Support Leads, OT's and Social Care Practitioners to achieve the best outcomes for people using the service.

A positive approach to risk taking, personal resilience and the ability to contribute to inter-agency and multi-disciplinary approaches, both across the Council and in other statutory bodies, are key to this role.

- Conduct person-centred assessments, working closely with individuals and their carers to gather information and positively influence wellbeing.
- Undertake personalised safeguarding enquiries, demonstrating strong communication and organizational skills.
- Collaborate with other organizations and professionals, fostering effective multi-disciplinary practice.
- Support individuals in maximizing opportunities for choice and control over their lives.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
