A fantastic opportunity has arisen for a Social Worker to join our Chesterfield Long Term Team.

Are you someone who enjoys working with long term clients in the community to support adults to maximise their independence?

Are you a creative problem solver who can work well on their own initiative and within a Long-term team?

If so, Derbyshire County Council Adult Social Care and Health wants to hear from you.  We are seeking to recruit a Social Worker permanent full-time based at The Quad in Chesterfield.  

As part of our Long-term team, you will be required to take a strength-based focused approach to deliver a proportionate, personalised, safe and sustainable response for local adults, their carers, and families. You must be able to work autonomously in a multi-disciplinary team. You’ll play a vital role in facilitating care act assessments for clients transitioning from children’s and 18’s and over, reviews in relation to community care homes and supported living schemes. You will actively investigate safeguarding concerns with initial inquires and S42. MDT working with professionals. There will be an expectation of you being part of a duty rota.

A positive approach to risk taking, personal resilience and the ability to contribute to inter-agency and multi-disciplinary approaches, both across the Council and in other statutory bodies, are key to this role.

  • Experience in assessment and reviewing is desirable.
  • Commitment to person-centred practice and a home-first approach aligned with the Best Lives initiative.
  • Strong organisational and time management skills.
  • Knowledge of local resources to support sustainable discharges.
  • Excellent communication and interpersonal skills skills to both support the people and their families, whilst enabling you to liaise and maintain positive relationships with hospital staff, CSB Unit staff and workers from other disciplines.
