We are excited to advertise an opportunity for a Social Worker to join the Out of Area Hospital Discharge Team (OOA HD), which is part of Derbyshire’s Countywide Hospital Discharge Service.
The team primarily focus on transfers of care for Derbyshire Residents from all Out of Area Acute & Private Hospital Trusts within the UK. The highest number of referrals come from trusts within Authorities that boarder Derbyshire including Greater Manchester, East Cheshire, Tameside, Sheffield, and Nottinghamshire.
The successful applicant is preferred to reside either within or bordering the mid – south of Derbyshire to assist attending, when required, provisions bordering the mid-south of the county such as Sheffield & Nottingham.
As this Service & post operates countywide, much of the work will be undertaken working remotely from home with some in-reach to the various hospitals when required. The successful applicant is preferred to reside either within or bordering the mid – south of Derbyshire. Base location to be determined depending on the successful applicant’s residence location.
The role entails proportionately assessing Derbyshire residents within Acute Hospitals to assist them via a strengths-based approach, underpinned by the Discharge to Assess Model to leave the acute with short term provisions such as informal services, homecare, therapy beds. The successful applicant would be expected to work autonomously & as part of a multi-disciplinary team.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
