***Passionate about delivering high-quality, strengths-based support in the community? We are looking for a motivated and experienced Social Care Practitioner (SCP) to join the North East Derbyshire (North) Adult Social Work Team***

Due to maternity leave within the team, we are currently considering applicants for a full-time, fixed term SCP position.

This is a great opportunity for someone wanting to make the move to a community-based, statutory adult social care role or increase their current hours across a fixed term. Our team is highly supportive and works on an agile, flexible basis between the office, at home, and across the NED (North) community (Dronfield, Killamarsh, Eckington).

Depending on service needs, there is an expectation to travel across the area and occasionally out of area. You will have the chance to work alongside experienced colleagues, plus link closely with health professionals and other partner agencies.

The successful applicant will need to demonstrate the skills, knowledge, and experience needed to hit the ground running in an area team and provide a safe, accountable, cost-effective service that helps clients achieve their outcomes. Experience is preferred, where essential role specific, mandatory training, support and 1:1, peer supervision will be provided. Please see the detailed job and person profile for more information.

Key responsibilities include carrying out assessments and reviews for individuals with less complex needs and arranging services to meet Care Act (2014) eligibility. You will be committed to working in a person-centred, strengths-based way with clients, carers, and families to maintain and maximise independence. This may involve home visits as well as work in residential and nursing settings.

The team has a dedicated duty system, though there may be occasions where you provide cover for incoming duty work, including triaging new referrals and supporting adults and carers who require an urgent response.
