An exciting opportunity has arisen to join our Derbyshire Dales North Adult Social Work Team as a Social Care Practitioner.
In this role, you will play a vital part in supporting adults within our community by providing professional triage and person-centred assessments of Adult Social Care needs.
We offer a hybrid working approach, giving you the flexibility to work both from the office and from home, balancing your needs with those of the service.
