We are a long-term generic adult team. The people we work with have social care needs resulting from a range of disabilities including learning disability, autism, physical disability, and dementia.  The successful candidate will have a working knowledge of the Care Act 2014 and Mental Capacity Act 2005 as well as safeguarding and experience of assessments.

The team is busy yet supportive with a strong team ethos based on traditional social work values with a passion for the wellbeing of workers. The social care practitioner joining us will need the ability to prioritise competing demands, be resilient and manage their time effectively. The role includes assessing and reviewing care needs and working with people and their support networks to create support plans. Using person centred, strengths-based approaches we support people to identify and achieve their goals with a home first approach.

The successful candidate will be given opportunity for daily support and peer group discussion and will be provided with regular professional supervision where you will be supported to achieve best practice. We operate a ‘champion’ role offering the opportunity to develop and share your knowledge and practice interests and weekly CPD sessions.
