Social Care Practitioner – Shared Lives Derbyshire

We are seeking a Social Care Practitioner who is passionate about working within a CQC‑registered service and who shares our belief that people are best supported within their own communities to live fulfilled, family‑centred lives.

The Shared Lives Derbyshire Scheme enables and monitors arrangements for adults who meet Adult Care eligibility criteria. Our service currently supports mainly adults with a learning disability and/or autism to live with approved Shared Lives carers in the carers family home. We are also keen to extend this model of support to other adults who may benefit from community‑based, family‑led care.

As part of the Shared Lives Team, you will support and guide Shared Lives carers to provide safe, high‑quality care within their own homes, with a strong emphasis on family life, community inclusion, and CQC compliance. Shared Lives arrangements may offer long‑term support, short breaks, or day support, all with the person’s wellbeing at the heart of the service.

We are responsible for ensuring people receive safe, caring, and meaningful support. We do this by recruiting, assessing, approving, training, and matching Shared Lives carers who have the right values, skills, and commitment to provide care in a place people can truly call home.

Although our team is within the Direct Care portfolio, this role does not include any physical care provision. Your primary role within our team will be supporting carers to deliver excellent care. Experience of working with people with a learning disability and/or autistic people is desirable, along with a strong understanding of the barriers and inequalities people with disabilities can face in accessing care, housing, and community life.

You will be highly motivated, able to drive and work across Derbyshire, and confident working collaboratively with carers, professionals, people using the service, and their families. This is an exciting time to join the service as we continue to grow and develop. The role is delivered through a hybrid model, with daytime weekday working.
