Amber Valley Short Term Assessment and Reablement Team are seeking a motivated, forward-thinking Occupational Therapist to join our dynamic Service. This is an exciting opportunity to work at the frontline of promoting independence, preventing hospital admission, and supporting timely, safe discharge home.

Our service provides rapid, person-centred assessment and reablement interventions for adults following a change in their health or functional ability. You will work collaboratively with colleagues across health, social care, voluntary sector partners, and most importantly individuals and their families to help people regain confidence and maximise their independence.

What You’ll Be Doing

  • Completing holistic, strengths-based assessments within people’s homes or community settings.
  • Developing goal-focused reablement plans that support functional improvement and reduce reliance on long-term services.
  • Working closely with Enablement Support Workers to ensure effective delivery of graded interventions.
  • Prescribing and recommending equipment and minor adaptations to enhance safety and independence.
  • Using your clinical reasoning to identify risk, promote wellbeing, and encourage positive risk-taking.
  • Maintaining clear, strengths-based documentation and contributing to multidisciplinary decision-making.
  • Involvement in Safeguarding Enquiries as required to support safety and wellbeing.
  • Undertaking Mental Capacity assessments as required.

About You

We’re looking for an experienced Occupational Therapist who:

  • Is HCPC registered and passionate about enabling independence.
  • Has strong assessment and problem‑solving skills.
  • Understands reablement principles, strengths‑based practice, and outcomes-focused support.
  • Works effectively within a fast‑paced multidisciplinary environment.
  • Can communicate confidently and build strong working relationships.
  • Values dignity, choice, and personalised support for every individual.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
