Are you passionate about making a difference in people’s lives? An exciting opportunity has arisen within the Brokerage Service for 1 dedicated Broker to join our Adult Social Care and Health team.
Brokering is the process of finding and matching providers with people’s specified support outcomes. The Brokerage Service is a dedicated team of staff covering the whole of Derbyshire, working with all adults and/or their representatives. People do not have to have had a Social Care assessment; or have been assessed as meeting eligibility criteria to access Brokerage support. Any person wishing to find or arrange support for themselves or a relative or friend may contact brokerage via Call Derbyshire/online web submission form/Health services etc.
As a Broker, you will play a vital role in arranging and coordinating care and support packages for adults, ensuring individuals receive the right services at the right time. The roles will involve liaising and negotiating with home care providers, care homes, customers and families, as well as working closely with Health/Adult Social Care colleagues, and provide feedback on outcomes achieved.
Key Responsibilities:
- Work closely with health and social care worker, service users, and providers to source and arrange care packages, care home placements, or other resource.
- Negotiate costs and ensure value for money.
- Maintain accurate records and ensure compliance with policies and procedures.
- Build strong relationships with care providers and internal teams.
- Support service users and their families/representatives to understand their options.
About You:
- Experience in adult social care, commissioning, or brokering (desirable but not essential).
- Excellent communication and negotiation skills.
- Organised, proactive, and able to manage a busy caseload.
- IT literate and comfortable with case management systems.
- Committed to promoting independence and choice for service users.
The vacancy is a permanent post, working on a Monday to Friday basis.
