An exciting opportunity has arisen within the Brokerage Service for a Broker who will focus on community and hospital discharge referrals, primarily sourcing home care support for people, but also supporting as required in sourcing other services such as care homes or supported living. The role will involve liaising and negotiating with care providers and families, as well as working closely with Health/Adult Social Care colleagues. A full induction and training will be provided.
The vacancy is on a Temporary fixed term contract until 28th February 2027 due to covering maternity leave, working on a Monday to Friday basis.
This post may suit a candidate who's post is at risk, or who is interested in a secondment in agreement with your line manager. We also welcome external applications but we may be expected to focus on offering to suitable priority at risk candidates first.
You will need to develop and maintain a good knowledge of services available, be able to negotiate details of service provision, act as a contact point, and be able to develop and maintain effective working relationships. The successful candidate will also be confident in the use of IT systems and processes, be able to maintain accurate records, prepare brief reports and statistics, and manage their own workload to meet deadlines.

Whilst the post will be initially based the Chesterfield QUAD during induction, we offer a hybrid approach to working, with our team currently working from home and from the office at least one day per week, to meet the needs of the service.
