Live Life Better Derbyshire (LLBD) is The Council’s lifestyle service and supports people to identify their holistic health and wellbeing needs, and provides weight management, smoking cessation and physical activity services across Derbyshire.

Live Life Better Derbyshire are looking to recruit 3 highly motivated Health Improvement Workers on a full-time basis:

2 positions will focus primarily on the Community Health Improvement element:

Position 1 will be based at Matlock covering the Derbyshire Dales/High Peak area.

Position 2 will be based at Swadlincote covering the South Derbyshire area.

 

The third position will focus on quality conversations:

Position 3 will be based at Chesterfield Royal Hospital but cover county wide.

Travel across the county will be a requirement of all positions. 

Position 1 & 2 – Community Health Improvement Worker

The successful candidates will support clients to complete our Health and Wellbeing assessment & Body MOT’s at a range of settings across the community and workplaces.  You will contribute to the health improvement of the population by supporting and signposting people to local organisations and services.  Strong partnership working skills will be required to ensure LLBD is embedded in the local community & will involve attending community events and engagement work.  You will work to support the NHS Health checks programme & CVD case finding initiatives. 

An excellent level of IT literacy including proficient use of Microsoft Word, Excel, Outlook and 365, along with the ability to adapt to bespoke databases is a key requirement of this role. It is also essential that the successful candidates have excellent communication skills with the ability to communicate with a wide range of people.  Awareness of barriers that may prevent a client accessing essential support or making a lifestyle change is key.

Position 3 Community Health Improvement Worker – Quality Conversations (stop smoking):

On 4 October 2023, the government published Stopping the start: our new plan to create a smokefree generation. This included a programme of funding to support current smokers to quit smoking, with £70 million additional funding per year for local authority stop smoking services and support. This additional funding will drive more people into stop smoking services and support more people to quit smoking.

There is an exciting opportunity to be part of an innovative project exploring whether having Quality Conversations can have an impact on patients who are waiting for elective surgery.

The purpose of this role is to contact patients on an elective surgical waiting list, who are living in the most deprived areas of Derbyshire, to have a quality conversation and help them to access services which they may benefit from.  The priority will be placed on those patients identified as people who smoke tobacco and support them into the Live Life Better Derbyshire stop smoking service.

The quality conversation approach helps patients to take an active role in improving their health and wellbeing.

All successful candidates must demonstrate the ability and commitment to undertaking a comprehensive range of duties as well as demonstrating flexibility in order to meet the demands of the service.  As the role involves dealing with sensitive information and vulnerable individuals it is essential that the successful candidates have a knowledge of confidentiality and GDPR.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
