The Preventing Family Breakdown Team is a countywide intervention service that aims to enable change within families who have a young person aged 10-17, with the young person being at risk of coming into care without extensive support. We also work closely with families who have a young person in care who has an identified care plan for a return home with intervention and support.
Our team is multi-disciplinary and consists of a Consultant Social Worker, a Clinical Psychologist, Senior Practitioner, Social Workers, Children’s Practitioners, Community Care Worker and Team Co-ordinator.
We currently have a fixed term full time vacancy, 37 hours per week for a Social Worker to work with families in the south of the county.
Due to the nature of our work, we hold a small caseload of families which enables intensive work to be undertaken.
