This is an exciting opportunity to join a team of Family Help 0-5 practitioners covering maternity leave, you will be operating as part of a targeted Family Help Service within the Amber Valley Locality.
You will be responsible for supporting the Family Help Senior Practitioner in the efficient and effective delivery of the 0-5 Family help service, demonstrating and championing evidenced based practice.
Your principle role will include:
Compiling action plans with measurable targets to co-ordinate support for families who are working with a variety of partner agencies.
- Tracking the individual progress of a child and cohorts of children where appropriate, measuring development and school readiness.
- Deliver specific intervention programs and evidenced based programs targeted towards children at risk of developmental delay such as the Every Child a Talker (EcaT) programme.
- Delivery of programme's to assist parents in their parenting, such as Solihull approach, understanding your child.
- Providing advice and signposting to parents/carers.
- keeping clear and concise records which are accurate on the councils recording systems.
- Report writing and completion of key documents such as safeguarding case conference reports to assist in collating essential evidence gathering to ensure decision can be made to keep children safe.
