⭐Social Worker – Adult Care (DART Team)⭐ We’re looking for two enthusiastic and motivated Social Workers to join our Discharge & Assessment Reviewing Team (DART). Our team plays a vital role in helping adults return home safely and live independently after hospital discharge.
