There is an exciting opportunity for a suitably qualified Social Worker to join the Chesterfield Mental Health Prevention & Personalisation Team for a part time position of 30 hours a week. You will be working within a very supportive and specialised mental health team alongside Social Workers and Approved Mental Health Professionals under the direct supervision of the Senior Practitioner and Service Manager.
Our specialist Mental Health Teamwork in close partnership with professionals from NHS Secondary Care mental health services to deliver Social Care support to people with the most complex needs in relation to a significant mental illness. Our aim is to offer short-term and preventative support to encourage independence, although we do carry several longer-term more complex cases who meet Care Act eligibility and require ongoing support.
There are also a number of Approved Mental Health Professionals in the team who assess people under the Mental Health Act 1983 (amended 2007) for detention in hospital and there will be opportunities for career development in this area.
The successful candidate will be familiar with Safeguarding, the Mental Health Act 1983, the Care Act 2014, and the Mental Capacity Act 2005. You will have experience of completing mental capacity assessments and making best interest’s decisions.
The team is busy and you will need to be able to prioritise competing demands and manage your time effectively. The role includes participating on the duty rota, offering advice and signposting, completing person centred assessments and risk assessments, and creating support plans. Person centred and strengths-based approaches form the basis of our ethos, and we strive to support people to identify and achieve their goals.
We cover the Chesterfield and Killamarsh area and under Derbyshire County Council’s Modern Ways of Working there will be a mixture of home and office-based working.
