We are looking for a social worker to join the Mental Health Hospital discharge team.

The post would cover the Hartington Unit in Chesterfield

We are looking for a social worker who excel in working in a multi disciplinary setting and are able to work from a social perspective in a medical environment

A good working knowledge of The Mental Health act is required specifically around 117 aftercare along with knowledge of the Care Act and The Mental Capacity Act.

Being qualified as an AMHP would be an advantage although not essential as there a opportunities for further personal development and the opportunity to qualify as a AMHP.

The Successful Candidate will be able to work autonomously and be confident in decision making, also be able to assess risk and ensure safe timely discharge.
