There is opportunity for a social worker to join Bolsover South P&P team. We work with vulnerable adults (and people approaching adulthood) with long term care needs living in the community and in care placements to complete a person-centred assessment of Care Act eligibility, including mental capacity assessment and best interest decision as required, and complete a strength-based support plan to meet their goals. The role includes visiting people at home, communicating with them and their carers to gather information, make decisions, arrange equipment and care, and record this. We undertake safeguarding enquiries using Making Safeguarding Personal.
We are social workers, social care practitioners, a senior practitioner, and a service manager. We work alongside other organisations and professionals including occupational therapists, and workers are linked to GP surgeries to aid effective multi-disciplinary practice.
There is opportunity for home and office working. Support is provided through a Microsoft Teams chat facility with the team, a daily Teams catch-up with team members and senior practitioner and/or service manager, monthly one to one supervision, and a monthly team meeting. We operate a ‘champion’ role, offering the opportunity to develop practice interests and share knowledge and with the team.
