We are currently looking for a Social Worker to join the Out of Area Hospital Discharge Team (OOA HD), which is part of Derbyshire’s Countywide Hospital Discharge Service.

The team primarily focus on transfers of care from Acute Hospital Trusts that boarder Derbyshire, including the Greater Manchester, East Cheshire, Tameside, Sheffield, and Nottinghamshire. As this post is countywide, much of the work will be undertaken working remotely from home with some in-reach to the various hospital when required. Base determined depending on the successful applicant’s residence location.

The purpose of the role will be to proportionately assess Derbyshire residents within Acute Hospitals to assist them via a strengths-based approach, underpinned by the Discharge to Assess Model to leave the acute with short term provisions utilising informal services, homecare, therapy beds & short placements. The successful applicant would be expected to work autonomously & as part of a multi-disciplinary team.

The successful candidate will be flexible and resilient, willing to adapt and continuously learn and cascade new skills and knowledge.  You will be able to work as part of a multi-disciplinary team and share a clear sense of purpose and vision with resilience and determination to bring about change.
