We have an exciting position open for a Social Worker to join the Chesterfield Royal Hospital Discharge Team on a fixed term basis to cover maternity leave. Experience of working in a hospital setting is beneficial but not essential, you will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people leaving hospital.
This post will require you work flexibly across hospitals north of the County to maintain system flow. The post is for a 5 in 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis.
The position requires on site working for the working week; with a rota for working from home.
You will need to be able to manage change and maintain positive relationship with health partners and other agencies. You will need to be committed to working in a person-centred and strength-based way with clients and their families to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance.
Due to this post having access to children and/or vulnerable adults, the successful candidate will be required to undertake a Disclosure and Barring Service check. The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
