An exciting opportunity has arisen and we a seeking to recruit an experienced Social Worker to join the Hospital Discharge Team for a fixed term period of 12 months (Covering Secondment), The role will be based at University Hospital Derby and Burton. Experience of working in a hospital setting beneficial but not essential, you will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people leaving hospital. This post will require you work flexibly across services and system partners to maintain flow. The post is for a 5 in 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. This this change prior notice will be given in advance.
You will need to be able to manage change and have the ability to maintain positive relationship with health partners and other agencies. You will need to be committed to working in a person-centred way with clients and their families to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance.
The successful candidate will be flexible and resilient, willing to adapt and continuously learn and cascade new skills and knowledge. You will be able to work as part of a multi-disciplinary team and share a clear sense of purpose and vision with resilience and determination to bring about change.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
