We are looking to establish a Mental Health Hospital discharge team.

There are three social work posts available - two full time and one part time (18.5 hours per week)

Funding is currently until the end of March 2024

The sites to be covered are Kingsway in Derby, Hartington in Chesterfield and Walton. 

We are looking for social workers who excel in working in a multi disciplinary setting and are able to work from a social perspective in a medical environment

A good working knowledge of The Mental Health act is required specifically around 117 aftercare along with knowledge of the Care Act and The Mental Capacity Act.

Being qualified as an AMHP would be an advantage although not essential 

The Successful Candidate will be able to work autonomously and be confident in decision making, also be able to assess risk and ensure safe timely discharge.
