We are looking to recruit qualified Social Workers to join our Adult Social Work Team, to work across the South of South Derbyshire.
We work with adults who have care and support needs (and people approaching adulthood) living in the community and in care placements to complete person-centred assessments.
This is a busy team based in the community and consists of Social Workers and Social Care Practitioners who work closely with partner agencies.
The successful applicant will be able to demonstrate the necessary skills, knowledge, and experience to undertake this role to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes. Key responsibilities will include carrying out Care Act (2014) assessments and reviews for individuals with complex needs as well as arranging the provision of services to meet eligible needs. It is expected that you will be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence.
We undertake personalised safeguarding enquiries where strong communication and organisational skills will be required. An understanding and experience of safeguarding is essential as well as experience of undertaking capacity assessments in accordance with the Mental Capacity Act (2005).
We work alongside other organisations and professionals and staff within the team are linked to GP surgeries to promote effective multi-disciplinary practice.
There is an expectation that you will act as duty worker on rota basis which will involve triaging new referrals to the team and support clients and carers who require a more urgent response.

There is opportunity for home and office working. Support is provided through a Microsoft Teams chat facility with the team, monthly one to one supervision, and a monthly team meeting.
