North East Derbyshire Adult Social Work Team are looking to recruit to the post of social care practitioner based at Clay Cross, Chesterfield.
This is an 18.5 hour part-time post. The required work pattern is 7.4 hours alternate Wednesdays, 7.4 hours Thursdays, 7.4 hours Fridays.
The team is based in the community and consists of Social Workers and Social Care Practitioners who work closely with partner agencies.
The successful applicant will be able to demonstrate the necessary skills, knowledge and experience to undertake this role in order to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes. Key responsibilities will include carrying out assessments and reviews for individuals with less complex needs as well as arranging the provision of services to meet eligible needs. It is expected that you will be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence.
There is an expectation that you will act as duty worker as required which will involve triaging new referrals to the team and support clients and carers who require a more urgent response.
You will need to be able to build positive relationships with health partners and other agencies.
