We are looking to recruit 3 enthusiastic and motivated Social Care Practitioners to join a new team within the Transformation and Partnership department in Adult Social Care. This is a full time, fixed term position up to 18 months.

As a Social Care Practitioner, you will be undertaking Care Act 2014 reviews and assessments with adults in the community who are in receipt of care, utilising a strengths-based approach which considers both the adult’s personal strengths and social and community networks. Focusing on maximising independence, choice and control will be central to the work you will do. 

You will have excellent communication skills to enable you to work effectively and in partnership with people, families, other professionals and agencies to promote person-centred outcomes for individuals with care and support needs. The role includes visiting people at home so the ability to travel is essential.

You can expect a robust induction which will include relevant learning opportunities with assessment and reviews, regular supervision and access to training opportunities to support your development in practice.

The work to be undertaken covers the whole of the county and you would be assigned an area to focus on.  This will be a hybrid working role, working from both home and a local office.  The specific areas are:

  • Amber Valley
  • Bolsover
  • Chesterfield
  • Derbyshire Dales
  • Erewash
  • High Peak
  • North East Derbyshire
  • South Derbyshire
