Social Care Practitioner

We are looking to recruit a Social Care Practitioner to join a busy personalisation and prevention adult care team working across the South of South Derbyshire. 

The successful candidate will have experience of working in an Adult Social Care environment to provide assessment and support under the Care Act (2014).  To be able to demonstrate an ability to work in a person centred and strengths-based approach to achieve the ideal outcomes with people will be an asset.  The candidate will need to have an awareness of the current legislation, policy and procedures that underpin good practice. The role includes the assessment and provision of equipment to support independence and safety in the home.

The successful candidate will contribute to the team’s responsibility to ensuring close working with partners in statutory and voluntary organisations to plan safe and timely discharges from acute and community hospitals.
