Derbyshire County Council has recently undertaken a detailed review of its Occupational Therapy services and are implementing a new countywide structure in January 2024. As a result, we are increasing our Occupational Therapy capacity by introducing a number of new and increased roles. We have a range of full and part time Occupational Therapy Social Care Practitioner opportunities across a number of localities in Derbyshire. 

We have 1.9 FTE posts in The North West & Dales team, one 0.5 FTE post in the North East team, and 3.4 FTE posts in the South team. We are happy to receive applications for a range of hours?

The successful candidates will undertake assessment and review of individuals with a focus on delivering a range of simple equipment and non-complex housing adaptations in line with preventative approaches. You will work with people and their families/carers to identify and mobilise their strengths, networks, communities and help to maximise independence and self-care. 

The role will work closely with other professionals such as Social Workers, Occupational Therapists and Occupational Therapy Coordinators and be an excellent communicator, having the ability to liaise with people and their representatives to gather or provide information, to support successful outcomes and provide a good quality customer focussed service. 

You will coordinate your own case load, ensuring the Major Adaptation and Disabled Facilities Grant (DFG) process runs smoothly with the support of the Occupational Therapy Coordinators and take action where there may be delays. The OT Social Care Practitioner will liaise with external stakeholders including District & Borough Councils around the administration of the DFG (Disabled Facilities Grant) and equipment providers. 

The Occupational Therapy Social care practitioner will be able to identify and assess personal daily living needs and take responsibility for planning and managing an individual workload.
