💬 What’s Living Well all about?
Living Well Derbyshire offers accessible mental health support 🤝 through a mix of healthcare professionals and voluntary sector partners. We focus on independence, wellbeing, and strengths-based care 💪.
👥 You’ll work with people who have:
- Primary mental health needs 🧠
- Learning disabilities 🧩
- Drug/alcohol dependencies 🍃
- Physical health needs ❤️

💼 Your Role as a Social Care Practitioner
👣 Be part of a multi-disciplinary team
📋 Deliver short-term, holistic assessments
🧭 Advocate for people’s social care needs
🗣️ Ensure the person’s voice is central to their care
🤝 Work closely with Derbyshire Adult Care & Living Well partners
📅 Fixed Term Until: 31/03/2026 (due to funding)
