Derbyshire County Council’s Living Well Team in Chesterfield is seeking a motivated and skilled Social Care Practitioner to support adults to achieve independence, wellbeing, and improved quality of life.
Working within a strengths-based, person-centred framework, you will support individuals with common mental health needs, helping them to remain independent within their communities and prevent reliance on long-term services. This role is central to our Living Well approach, focusing on early intervention, prevention, and enabling people to live fulfilling lives.
Key Responsibilities
- Undertake person centred assessments
- Work collaboratively with individuals, families, carers, and partner agencies
- Promote independence, resilience, and community connections
- Manage a caseload
- Support safeguarding enquiries and contribute to risk management plans
- Signpost and connect people to community and voluntary sector services
- Maintain accurate and timely case records in line with organisational standards
About You
We are looking for someone who:
- Has experience working in adult social care, health, or a related field
- Demonstrates a strong understanding of person-centred and strengths-based practice
- Has knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding frameworks
- Is confident in managing competing priorities and working autonomously
- Has excellent communication and partnership-working skills
- Is committed to promoting dignity, independence, and wellbeing
A relevant qualification in health or social care (e.g. NVQ Level 3 or equivalent) is desirable.
