An exciting opportunity has arisen for two fixed term Social Care Practitioners to join the CSB & CH Hospital Discharge Team primarily to help support the flow of Community Support Beds (CSBs) within DCC residential care homes. The posts will initially be for 6 months from the date of appointment, with the possibility of extension or becoming permanent posts, dependent on funding.

One of the main functions of the CSBs is to provide assessment, intermediate care and therapy for people being discharged from hospital with a focus on person centred goal setting and improving independence, subject to continual assessment and timely review, so that the person can return to the place they call home, whenever possible. The post holders will be key players in supporting health and care systems during a period of increasing pressure on hospital beds and supporting the management of change as the CSB provision transitions to single operating models at three dedicated CSB units. These will be located at Bennerley Fields in Ilkeston, the Staveley Centre in Chesterfield and Thomas Fields in Buxton.

The focus of each post and base will be agreed on appointment and located as close to your home as possible to facilitate smarter travel. A flexible approach is required as you may also need to support with discharges from Community Hospitals to provide cover for absent colleagues.

You will need experience of assessment and reviewing with a commitment to person centred practice, taking a home first approach in line with the Best Lives initiative. You will need good organisational and time management skills together with a knowledge of local resources, to achieve timely discharges and positive outcomes for the people in the CSBs. You will also require excellent communication and inter-personal skills to both support the people and their families, whilst enabling you to liaise and maintain positive relationships with care home staff and workers from other disciplines.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
