An exciting opportunity has arisen for a suitably qualified Senior Practitioner to join Living Well Derbyshire which is a new collaborative programme currently being rolled out across the county.  This position will be based in Amber Valley. Focussed on helping local people to recover from mental health illness, the programme aims to offer more accessible support by which to promote a person’s mental health and wellbeing. The people you work with will have primary mental health needs, but may include people with a learning disability, drug or alcohol dependency and physical health needs. As a Senior Practitioner you will be a qualified and registered Social Worker.  You will be working as part of a multi-disciplinary team alongside other Living Well partners to offer holistic and joined up care and support.  Your focus will be to identify and advocate for people’s social care needs. You will be actively involved in complex case work and will provide professional leadership and advice (to include formal supervision) to Adult Social Care colleagues within the Living Well programme and more generally to support the work of the multi-disciplinary team around you.

As Senior Practitioner (Adult Social Care) you will be a key link between Derbyshire Adult Care and the Living Well programme, ensuring consistency of service and support on a local and countywide level.  You will work with people and carers who use our services to continuously seek and engage lived experience input, to ensure the person’s voice remains paramount to both their care and the shaping of the developing service.

You will make a significant contribution in the development of Adult Social Care’s role within the Living Well team. Applying a strengths-based approach to your practice you will be focussing on co-production with people who use our service and carers to identify and meet the needs of individuals, whilst taking wider development action to upskill and empower the local community, including its residents and partner organisations.

You will have a key role in understanding and contributing to changes and development of the Living Well programme, as such you will be confident in managing uncertainty and supporting colleagues through change. You will be adaptable and willing to feed into design discussions of the programme with multi-agency colleagues, in addition you will support the implementation of the Living Well programme to colleagues internal and external to the Adult Social Care department to include the development and facilitation of peer support sessions across the Living Well programme.

Reporting to a Service Manager within the departments Mental Health division your role will be to supervise social care staff and lead on professional practice.  You will support Adult Social Care colleagues both within the local Living Well team and on a countywide level including identification of training and development needs with focus on an ethos of empowerment and co-production to reduce need in these areas.   

This position is part of the Amber Valley, Bolsover and Clay Cross Mental Health Prevention and Personalisation Team which is primarily based in Clay Cross and the successful applicant will be expected to work from the Clay Cross office when necessary.
