We are looking for a passionate, person-centred Occupational Therapist to join our Erewash Short Term Assessment and Reablement Team (START). This is a permanent, full-time opportunity based in Erewash, offering a varied and rewarding caseload.

About the Team

Erewash START provides a multi-disciplinary approach to assess, support, and enable adults to maximise their independence and identify any potential long-term care needs. We are committed to working in a person-centred and strengths-based way, recognising people’s assets and enabling them to reach their goals. While community-based, the team also supports individuals transitioning from residential and nursing establishments and facilitates safe and timely hospital discharges. We offer a hybrid working model, with staff working from home and the office depending on service needs.

About the Role

We want to hear from someone who can:

• Deliver strengths-based, holistic assessments to support adults, carers, and families.
• Support the review of goals and progress against these using feedback from the MDT
• Promote independence, choice, and wellbeing through creative problem-solving and personalised interventions.
• Work across a variety of areas including safeguarding, complex health and social care needs, moving and handling, learning disabilities, assistive technology, hospital liaison, and adaptations.
• Assess for and provide equipment and minor adaptations as part of a full assessment
• Complete risk assessments and risk enablement plans to support people to remain as independent as they can safely be.
• Assess and determine suitability for single-handed care approaches
• Collaborate with a multidisciplinary team and contribute to inter-agency working across health and social care.
• Assess and make recommendations for the provision of major adaptations which will be referred to the OT co-coordinator.
• Offer expert advice and support to the multidisciplinary team
• Refer and signpost to other services as appropriate.
• Be part of a forward-thinking team that’s shaping the future of reablement in Derbyshire

Who We are Looking For

The ideal person will be:

• HCPC registered (or have applied for registration).
• Flexible, approachable, and an excellent communicator.
• Experienced or have excellent knowledge of:
  • Person-centered, strengths-based practice
  • Assessing daily living activities and occupational performance
  • Working with physical, learning disabilities and/or mental health conditions
  • Equipment and adaptation solutions.
  • Reablement approaches
  • Safeguarding principles and statutory legislation in adult social care.
• Confident managing and prioritising your caseload.
  A reflective practitioner committed to supervision and professional development.
  Passionate about sharing knowledge and supporting others to learn and grow.
