🌐 Area of work: Across High Peak and Derbyshire Dales

 As an Enablement Worker, your key responsibilities include:

To provide a wide range of practical, social, and general care services for individuals using the enablement service, in line with the agreed Personal Service Plan.

Alert managers to changes in presenting needs and respond in emergency situations liaising with colleagues, other agencies, and carers as appropriate.

Support and encourage individuals to maximise independence through engagement in social activity, working with local community networks, voluntary and self-help groups.

Complete records in respect of, Personal Service Plans case notes.

Liaise with colleagues, voluntary bodies, health service colleagues and other agencies, working with people with a range of complex needs and their families/carers, ensuring the safeguarding of vulnerable adults.

Maintain confidentiality and adhere to the departments recording systems.

Additional Highlights:

🛣️ Travel Required: Explore the High Peak and Derbyshire Dales area and occasional trips to other Derbyshire locations for training and meetings.
