A vacancy has arisen for a full-time community care worker to work in a busy, supportive and dynamic fieldwork team supporting people from 18 (or approaching 18) and their Carers.

The successful candidate will be based within the South Personalisation and Prevention fieldwork team.

The role includes assessing and reviewing the care needs of those in the community, those in care homes and discharges from hospital. The successful candidate will be required to assess and commission orders for non-complex equipment and property adaptations to maintain the independence of adults within the community.

We value working with people and their support networks to create support plans, using person centred, strengths-based and home first approaches to enable people to identify and achieve their outcomes in life. 

We continue to have hybrid working from home and have limited access to modern office space. There will be an expectation that the successful applicant is on a rota basis for duty and joins their colleagues in base as this offers peer support and advice to one another.

The successful candidate will be given opportunity for daily support and peer group discussions and will be provided with regular one to one supervision.
