This is an exciting opportunity to join the Shared Lives Team, to assist the Scheme Manager in the delivery of Derbyshire Shared Lives scheme for over 18’s that meet the Adult Care Criteria.

The role includes:

  • Supporting and monitoring a caseload of Shared Lives carers to meet the requirements of the scheme and the Health and Social Care Act (2008), Care Act (2014) and other key legislation and CQC guidelines with regards to services that offer personal care
  • Supporting a caseload of Shared Lives carers through telephone contact, online and home visits, dealing appropriately with any problems that may arise during an arrangement and undertaking annual Shared Lives carer reviews
  • Participating fully in the referral, allocation, assessment, and matching process
  • Promoting the services offered through networking and promotional events
  • Ensuring the wellbeing and safety of people using the service by producing high quality Personal Service Plans, risk assessments and risk enablement plans

You will need good organisational and time management skills together with excellent communication and inter-personal skills to both support and maintain positive relationships with our Approved Shared Lives carers. 

Our workers cover the whole of the County, and our base is Ripley Area Office.
