The Enhanced Support Team which is based in Staveley is looking for a full time, fixed term (6 months with a possibility to extend, subject to grant funding approval) Community Care Worker to join our team.  Working with adults with learning disabilities and/or autism our team works with people whose complex presentation requires highly specialist assessment and care planning skills that support individuals to live satisfying lives in the community.

This is an exciting but sometimes challenging opportunity for the right person, and the ability to work with adults with learning disabilities and/or autism is a key requirement of the post.

This is a newly created post which we anticipate will play an integral role within the delivery of the ‘Building the Right Support’ agenda in Derbyshire.  It is intended that the successful applicant will work alongside the team of Social Workers and a key aspect of the role will be to support the identification of appropriate supported living and residential placements for people known to our team – those discharging from hospital and those whose current arrangements are no longer suitable for them.  Working alongside health colleagues and other partners this will be a good opportunity to develop your collaborative working and negotiating skills. 

This is a great opportunity to develop your professional practice working with people with a learning disability and/or autism who display behaviour that challenges, to include self-injurious or aggressive behaviour and behaviour which can lead to contact with the criminal justice system. 

Whilst the team is based in Staveley, we support people throughout Derbyshire therefore we are happy to discuss working arrangements, to include working from home.
