South Derbyshire Active Communities and Health Team are looking to recruit a highly motivated Active Communities and Health Coordinator to support various projects across the District of South Derbyshire. 

The role is for 30 hours and will include daytime and evening hours subject to the demands of the service.

The role will be responsible for the coordination, delivery & monitoring of high-quality support and intervention programmes to a variety of audiences across South Derbyshire. 

You will lead on the coordination and delivery of Sports Mentoring, providing one to one mentoring sessions to young people aged 8-16 who have been referred into the programme for reasons such as poor school attainment or attendance, involvement with anti-social behaviour or poor mental health, using sport and physical activity as a vehicle for change. 

And Childs Choice, providing bespoke support to families in South Derbyshire. Children aged 5 to 12-years-old who are over a healthy weight or at risk of becoming an unhealthy weight will be eligible to access support in finding local opportunities to increase the child’s and/or families physical activity levels. This role will be responsible for identifying suitable opportunities based on their needs and supporting the family to access these opportunities, removing any potential barriers were possible and working to ensure participation is sustainable. 

Due to the nature of this role, the post holder will have experience of delivering to young people with complex needs and behavioural issues and experience of coordinating, delivering and reporting on externally funded projects in line with Service Level Agreements. 

This role will also support South Derbyshire District Council’s extensive range of community-based programmes within the Active Communities and Health Team.
