We are seeking to recruit a Social Worker to join the Out of Area Hospital Discharge Team. For this post the focus will be to support Derbyshire residents to be discharged from Acute hospitals bordering the mid & south of the county. This will entail developing effective working relationships with Hospitals such as the Nottingham University Hospitals, Doncaster Hospital and Bassetlaw Hospital. There is an expectation to aid covering other hospitals when required, as a team we work flexibly to manage referral demand across our service.  

The team currently work remotely predominantly. There are opportunities to access various Derbyshire offices within the county & hospitals outside of the county. There is an expectation to attend hospitals, when proportionate to aid assessment & patient flow.  

Experience of working in a hospital setting is beneficial but not essential. You will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people leaving hospital utilising a Home First approach & the Discharge to Assess model. This post will require you work flexibly across several hospitals including out of area hospitals to maintain system flow and timely discharges. The post is for a 5 in 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. Prior notice will be given in advance if this changes.  

You will need to be able to manage and positively adopt change. You would be expected to work effectively and build positive relationships with health partners and other agencies. You will need to be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
