To lead the Adult Care response, through both the leadership of several teams which support people to be discharged from hospital or avoid an unnecessary admission, and through wider collaboration with Council partners, to ensure there are effective arrangements which enable Derbyshire people to receive the right care, at the right time and in the right place. Working collaboratively across the system, with Partners and Colleagues to provide the best outcomes for people across the County.

What will I be doing?
You’ll be leading or (be part of) one of the teams who are embedding and shaping our Adult Social Care offer. You can work flexibly, and some of our teams have opportunities for remote working; our technology platform and equipment are first class enabling you to connect and collaborate with colleagues.
