Job Title:

Community Care Worker

Purpose of the Post

The routine visiting and assessment of those persons requiring help, and the provision of such assistance under the direction of a Service Manager and in conjunction with Care Managers.

Accountable To

Area Manager and relevant Service Managers

Supervisory Responsibilities


Duties and Responsibilities - Specific

To undertake complete holistic assessments of service user needs on less complex cases, to include a range of outcomes including community and residential services, in whatever service user group.
To undertake reviews of service users and their care plans.
To commission services both internally and externally.
Arranging for the admission of service users to local authority and independent sector residential care homes, nursing homes and day services.
Provision of community care packages for support to people in their own homes.
Providing information about the community care services available to people, the eligibility criteria and procedures for accessing and obtaining services.
The delivery of home equipment as appropriate.
Assisting Care Managers/Social Workers and others in the implementation, co-ordination, monitoring and reviewing of care/treatment plans.
Arranging/providing transport for or acting as escort to clients.
Liaison with voluntary bodies, residential and day establishments and other
Serving on ad hoc bodies/working parties etc.
Recording of all work undertaken and the preparation of reports/written assessments, care plans and reviews.
Assisting with group and community projects in the area as appropriate.
To undertake such training as is required.
To maintain confidentiality and observe data protection guidelines.
To carry out any other reasonable duties within the overall function, commensurate with the grading and level of responsibility of the post.

Duties and Responsibilities - General

To adhere to existing working practices, methods, procedures, undertake relevant training and development activities and to respond positively to new and alternative systems.
It will be necessary to work with information technology and associated systems in accordance with County Council policies.
To co-operate with the council in complying with relevant health and safety legislation, policies and procedures in the performance of the duties of the post.
To carry out the duties and responsibilities of the post in compliance with the County Council's equal opportunities policies.
To maintain confidentiality and observe data protection and associated guidelines where appropriate.
To understand and comply with the County Council's environmental policies.
To carry out any other reasonable duties and responsibilities within the overall function, commensurate with the grading and level of responsibilities of the post.
